My aunt stayed here for a few weeks and the staff is amazing, friendly and caring. It starts from Nurses whom are on top of the care, CNAs that are on top of daily care, housekeeping in keeping the place clean. My aunt loved the food. Any issues that would happen the staff was there, they answered every question we had and concern. If I can give them 10 starts I would, . This place is highly recommended for anyone needing care the Meadows is a top notch.

The Meadows Post Acute in Panorama City is a place of healing and improving QoL (Quality of Life). It is run by a very efficient administrator, Khristine B, who personally interacts with families and ensures that the patient receives every thing to make their stay comfortable. The Dir of Nursing, Dir of Rehab, Dir of Staff development, Dir of Infection Prevention Dir Of Admissions, Social Workers, charge nurses, support staff, Activities Supervisor, Kitchen Supervisor, Facilities and ancillary staff are cheerful, compassionate, polite and very prompt. This team is fantastic - we always felt welcome during visiting hours. A special thanks to the evening and night shift - their personal touch made a vast difference. During rare down time, they even sat down and talked to patients and family. We never felt alone during our stay. Rather we felt surrounded by close friends. This team ensured that my dad received the best care possible; recovered; and was healthier leaving the facility than when he came in. Sincere thanks to the amazing staff of Meadows, Panorama City.

I had my family member stayed here and the employees were so nice and welcoming. The administrator Khristine was very accomodating, made sure my family member was taken care of. I recommend this place.

My mother’s experience at this rehabilitation center was disappointing and concerning. Unfortunately, I observed neglectful behavior towards patients, especially the elderly and patients without advocacy. I became aware of falsification of medical records when I asked to see her prescription medication, which was still in a sealed package. It was evident that her declining health was a direct result of the nurses not giving her the medicine, but logging and signing the medication records. How criminal and dangerous! The most troubling aspect is that the administration and supervising nurses are fully aware of the issue. If you or a family member are admitted at The Meadows, ensure that you are constantly vigilant that the medications are administered as prescribed. The overall atmosphere felt lacking in care and professionalism. I strongly advise others to consider alternative options for rehabilitation services.

This place is very corrupted, they change signing sheets, don’t give medication and ignore patients. Their behavior changes when there is visitors . Also when there incidents they make no report . This place should be under watch because all they want is collecting money . Called and asked for a patient and they said the patient is there but the patient was not there…. Shame

I am a licensed psychologist that has been working here for 3 years I see almost all the residents. I am blessed to work here, I work with 14 other facilities this is by far the best. Sandra the DON is amazing everytime I address a medical and psychological issue she is on top of it with care and compassion. The administrator Christine knows all the residents and personally visits everyone to address any concerns. Brenda and Mark the SW are on top of the facility checking in with the residents needs all the time and very attentive. They really care for the needs of every resident. The activities director Ingrid knows every resident and tries to make them feel safe and busy with activities. This place is the best which makes my job at the facility easy. The facility is always clean and comfortable. The staff here work amazing as a team. I would trust my family here.

Great facility with top notch care providers… My elderly friend received excellent care at The Meadows Post Acute for the months he was there. Everyone involved was extremely communicative and kept me up to date my friend’s health. The staff was very nice and genuinely seemed to care about the patients and those who came to visit. One thing that particularly impressed me about Meadows was that whenever I would call, if my friend did not pick up his phone, someone would actually go to his room to see why not and try to get him on the phone. I have visited my friend at all different hours, and they are consistent. Facility was clean and tidy. If any of my Family members need nursing care, I will definitely have them come here!

Very bad experience . The front desk wasnt helpful at all she was very rude . And also after a few times calling i got the director of staff development, DSD . And make appointment with him . I was on time . After I did my application and ask 4 interview, he said he has meeting . Whats the point making appointment ? He doesnt know thats 4 sure and the facility rely on him . Very sad . 😞😞😞

Staff has been incredible in taking care patients. Facility is clean proper and very friendly. The main focus of physical therapy and getting back to health makes it easy to accomplish at The Meadows Post-acute care. I feel safe with family members that have surgeries or need rehabilitation to come here!

The Meadows had many problems when Sheri Reyes was the Administrator but as soon as they brought in a new one its becoming a GREAT place to live and rehabilitate.I recommend it for all patients young and old.

I just met with my sons care team and can only say that a boulder was removed from my shoulders. They are so caring, empathetic and warm! They answered all of my questions and never made me feel rushed. The grounds are lovely and welcoming. If my son must be in a medical care facility, i thank heaven that hes at The Meadows.

My mom was brought there from the hospital after the stroke. We were on video call almost 24/7 using her tablet. Dont get me wrong. There are good staff there whom I met but in general, i just can not give this place 3 stars because there were more frustrations and disappointments in the care of my mom. The entire time she was there which was 2 months, she was neglected several times, a call light which was not answered for over 1 hour which happened several times, 2 meals missed after insulin was given a dinner and a breakfast. It was given only 2 hours after i notified the supervisor. She missed a PT day for reasons that were an easy fix. Some staff were rude too. There are more things that did not need mentioning. When the case manager notified me that the Physical Therapy was over but she can stay there as a nursing home resident, I obviously took her home because I could give her better care. OMG, when I took her home, her hair was so tangled. It took me more than 1 hour to fix it. Her skin was so dry. Its been over a month since she got home. She is a lot stronger now, more stable walking, her left side is stronger. I do PT, OT, ST exercises 4 days a week with her for at least 2 hours a day. The PT, OT comes 2 times a week. Thats a total of six days of therapy a week. Sunday is her rest day. However, for the good staff who really cared for mom i give them 5 stars. My mom mentioned a nurse who was older who took care of her really well but she doesnt remember the name, to you and the good ones, nurses, nurse aides, PT, whoever else, you know who you are if you cared wholeheartedly, WE thank you. I work in healthcare for more than 30 years now. I do management, research, education, bedside. Im very familiar with this field of work.

Outstanding, attentive, and caring nursing staff and administration. It’s by far the best rehab facility in the Valley. Excellent primary care and rehab physicians are closely involved in the care and progress of each and every patient. The facility is super clean and spotless. You’re loved one can expect the best in care at Meadows Post Acute.

I cant even begin to explain the esperiance I have been having with this horrible place. They treat the patients like prisoners. They only care about money the director of the facility hides behind closed doors in her office if you try to talk to her to adress any issuesyou might have and the worst is Monique the so called Administraror. Shes very aggressive and thinks she knows the Law. trying to squeeze every penny out of the patients and their families. Claiming since they provide all the care they are entitled to get every penny they can. Out of the families When in reality they have the patients starving sometimes they serve very little food . Or the food is so terrible the poor patients cant even eat it. So if you really love your family dont even consider this awful place for the ones you love.

My brother has been in this facility for almost 2 months now. He is always saying how nice the nurses and staff are there. It is tough having to accept living in a facility for awhile but it certainly helps when you like the people around you.

My son David was at the facility for 23 days of physical and occupational theraphy. The nurses and staff were very accommodating and friendly. I also want to thank to thank Nico the social worker who answered all my questions and made me feel that he was in a safe place. Thank you again and God Bless. His Mom Patty Strickroth.

No Stars! I had to give one or it would not post.Absolutely horrible, run as fast as you can from this place!Staff are never available, do not return calls, it is almost impossible to get anyone to answer the phone. Residents have had to call 911 because no one will answer call buttons. My husband was waiting hours for an ambulance to take him to the hospital and they would not allow me to see him. There is not enough room to write everything down…..they certainly shouldn’t allowed to be licensed and need to be shut down!

This is my initial assessment covering my Mothers Transfer process, and the first two days of her inpatient care. The Facility is newly renovated.It is very bright, airy, and clean smelling, in general. There is a nice outside sitting area. I have found all the staff very accommodating. Threy were able to approve and transfer my Mother to their facility with exxpediency and ease. They are striving for excellence. With new owners, the practices and procedures for quality control and patient care are evident. The medical attentiveness was impressive. Rather than seeking to put a cathetor in, in order to limit the nursing assistance that may be required in helping mom go to the bathroom, they were concerned as to how quickly; If not immediately, the cathetor could be removed. Her attending Physician at Mission Community Hospital is also able to be her attending Doctor during rehab. Therefore, her medical history, conditions, and needs are well know. So, her very good care at the hospital is carried over to the rehab to be continued in care during rehab. The Chef made several personal visits to my moms room to gain an understanding of her likes and dislikes within her prescribed diet. They want her to enjoy food and to be able provide her with meals that meets her needs My Mother had had major surgery requiring life support, followed by two additional hospital stays, one of which where she was back on life support. The Rehab Department wasted no time in evaluating her and getting her sitting up and standing. With l three Rehab persons working with her together simultaneous, I feel confident she will be able go home in a few weeks andfulky care for herself,again. They also have a dedicated staff member assigned to visit each patient everyday to ask how things are going for them. They want to know if all needs are being taken care of, nurses response times quick enough, hows food, how are they feeling, ect. They arranged for my sister to sleep adjacent to my moms bed for the night she was here out of town. The staff assisted as liaison with an in facility dental visit.Also, assessed and requested Wound Care and Pain Mgmt Specialist. The patient s were not just in their rooms all day . A majority were up and about in therapy or care of some sort, involved in a scheduled activity or enjoying g sitting in the outside garden area. The a rehab room was active with many patients and many therapists. Several patients were practicing walking down the halls with assistance. Sost of the facilities I had visted had no one doing anything anywhere. The Staff to Patuentratio seemed very high. They have excellent food. Home made cookies were baked and served warm to each patient where ever they were in the building. In the afternoon, fresh smoothies were made for everyone and personally delivered to the patients, as well. They have a parking lot behind the building where valet parking is provided. The lot is on the smaller side, butut worjedoutfibe The only I convenience was having to walk entirely around the building to the front entrance. I understand entering via the back entrance may pose a security risk,, however I recommend a second staff be hired or assigned to the back entrance so family members wont need to walk around each time they come and I, which may be several times per day. Staff were very nice and helpful. Overall, Excellent!!!!!

First of all Dr. Singh is so kind and so intelligent. He talks to me about my condition and helps guide me to my rehabilitation. It is a long and painful road but I know I can trust him.I am a patient here and now. The staff is working so hard. We can be difficult to take care of and I dont know how they get it all done. Thank you to everyone at The Meadows for taking me in when Im at my worst.

Dont bring your family members here if you love them. My sister was there overnight and the staff stole her stuff. Envelope of money and gifts that were purchased for her. Guess what they returned ..paperwork. They stole her money and gifts when I went there they were all scooting about because they knew they STOLE from a CANCER PATIENT. Please dont bring your family here if you do dont bring anything of value...they will steal it.Under my review is a moron from there telling me sorry about our experience . They strive for quality....what a colossal joke. Do you mean the experience of your hoodrat caregivers that rummaged through my sisters stuff and took what they can use. Disgusting. My sister has passed away since and I will never forget how this place ROBBED A CANCER PATIENT! And they tell me now...sorry for Your experience here. Disgusting behavior from so called professionals. DONT BRING your family here if you care about them.

Every time I tried calling to speak to a nurse or social worker, I am on hold for 45 minutes!! They do not take patient care very seriously. We have had one patient with them and I will never refer anyone else in the future. I am guessing they are way under staffed. They need to get it together. :(

I went to go visit my cousin and noticed everyone has bad customer service. Right when I walked in I went to the front desk and everyone was just walking passed me or just not looking at me .. I was not attended at all.. Then when I was with my cousin she wanted some help so she pressed the button to call them and no one showed up 5 minutes passed and still no one showed up so we had to call them .. Poor service I would rate it 0 stars if I could
