I went to get a notarized affidavit for name change for Indian passport. Notary service was quite hassle free, and Meena mam, also did the additional work of posting my VFS application, at UPS courier. The only issue is, she wasnt aware of the process for complex name change, i.e. newspaper ads, which caused a slight delay. But mam was very helpful in providing support, whenever I needed.
